This form is an important legal document. It explains the risks you are assuming by participating in the Cliff Cruise 2023 and any activities surrounding it. It is critical that you read and understand it completely.
After you have done so, please fill in the necessary boxes below. By doing so, you are agreeing to all the terms of this document.
If under the age of 18, a parent or legal guardian must complete the form on behalf of the minor.

Waiver, Informed Consent, and Covenant Not to Sue

I have volunteered to participate in the Cliff Cruise 2023. This event is not mandatory, and by signing this form, I understand that I enter into it without coercion. I do here and forever release, discharge, and hereby hold harmless Extreme Vertigo LLC, Travis Sims, Døds Federation, City of Chattanooga, Watershow Productions Incorporated and any other related parties, respective agents, heirs, associates, contractors, and employees from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my participation in this event including any injuries or legal issues resulting therefrom. THIS WAIVER AND RELEASE OF LIABILITY INCLUDES, WITHOUT LIMITATION, INJURIES OR LEGAL ISSUES WHICH MAY OCCUR AS A RESULT OF (1) ANY ACCIDENTS RESULTING FROM WALKING, CLIMBING, JUMPING, DIVING, FLIPPING OFF CLIFFS, BRIDGES, PIERS, STRUCTURES, TREES OR ANY OTHER PLATFORMS DURING THE DURATION OF THE EVENT. (2) ANY ACCIDENT; SLIP, FALL, OF MYSELF OR OTHERS WALKING, HIKING, OR CLIMBING ANYWHERE DURING THE EVENT (3) ANY ACCIDENT, VEHICLE DAMAGE, TRAFFIC OR PARKING VIOLATIONS BEFORE, DURING OR AFTER THE EVENT. (4) ANY LEGAL REPERCUSSIONS FOR BREAKING ANY POSTED LAWS DURING THE EVENT.
Assumption of Risk

I recognize that participating in this event and any activities involved with it can be dangerous and injuries/ death can occur. I acknowledge that there are inherent risks in hiking, climbing and jumping off anything before, during or after the Cliff Cruise Event.
I certify that I have either consulted with a physician, or that I am certain of my own general good health that I can participate in such an event without a significant risk of injury or harm to myself or the health of anyone else.

I acknowledge and agree that I assume the risks associated with any and all activities in which I participate.
I acknowledge that there are inherent risks of drowning and that the river current and depth can be dangerous, and I am responsible for my safety.
I acknowledge and agree that no doctor or medical professional has or would recommend me against participating in any activities involved with the Cliff Cruise. I understand that results are individual and may vary.
I acknowledge that there may be incentives/prizes, however that will not affect my ability to reasonably make safe and smart decisions.
I acknowledge that everything I do before, during and after the Cliff Cruise, is at my own risk and cannot hold Travis Sims, Extreme Vertigo LLC, City Of Chattanooga, Døds Federation, Watershow Productions Incorporated, or any affiliated parties liable.
I ACKNOWLEDGE THAT I HAVE THOROUGHLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. BY SIGNING THIS DOCUMENT, I AM WAIVING ANY RIGHT I OR MY SUCCESSORS, FAMILY, OR RELATED PARTIES MIGHT HAVE TO BRING A LEGAL ACTION OR ASSERT A CLAIM AGAINST EXTREME VERTIGO LLC, TRAVIS SIMS, THE CITY OF CHATTANOOGA OR ANY PREVIOUSLY MENTIONED ASSOCIATES , REPRESENTATIVES, OR SUCCESSORS INVOLVED, PAST, PRESENT OR FUTURE FOR ITS NEGLIGENCE, MY OWN NEGLIGENCE, OR THAT OF ANY EMPLOYEES, AGENTS, ATTENDEES OR CONTRACTORS INVOLVED.
Crowd Filming Notice / Release

Please be aware that by participating in the Cliff Cruise or any activities involved, you acknowledge that your voice, name, and/or likeness may be used, without compensation or approval, in video and photography for use in any and all media, whether now known or hereafter devised, for eternity, and you release Extreme Vertigo LLC, Travis Sims, City Of Chattanooga Watershow Productions Incorporated, or any partners, successors, associates or licensees from any liability whatsoever of any nature.
